Privacy Policy

Pulmonology Billing Services (“PBS,” “we,” “us,” or “our”) operates https://pulmonologybillingservice.com and its associated pages, content, and quote-request features (collectively, the “Site”). This Privacy Policy explains how personal information is collected, used, disclosed, and otherwise processed in connection with the Site and describes the choices and rights available to individuals in the United States.

The Site is a business-to-business lead-generation service for physicians, medical practices, hospitals, and other healthcare service providers seeking quotes from independent medical-billing companies. PBS does not itself provide medical-billing services through the Site.

By using the Site, you acknowledge the practices described in this Privacy Policy. If you do not agree with this Privacy Policy, do not use the Site or submit a quote request.

1. Scope

This Privacy Policy applies to personal information processed in connection with the Site. It does not apply to information handled independently by 360Connect, LLC (“360Connect”), our form and lead-processing provider; a billing provider that receives a verified lead; Google; or another third party under that party’s own privacy policy, except to the extent PBS determines the purposes and means of the processing.

The Site is intended only for users in the United States who are at least 18 years old and who act for a healthcare service provider or other business. It is not intended for patients, children, or people located outside the United States.

2. Important Summary of the Quote Process

When you submit a quote request through the Site:

  1. the form information is collected and processed through 360Connect;
  2. 360Connect may review or contact you to verify the request and obtain information reasonably necessary to match the request;
  3. after verification, the request may be disclosed to as many as five relevant, independent medical-billing providers;
  4. those billing providers pay for access to the lead and may contact you to gather additional information and prepare a quote; and
  5. each billing provider handles the information it receives under its own privacy practices and is not controlled by PBS.

PBS does not maintain a separate database or copy of quote-request submissions. However, 360Connect, the billing providers that receive a lead, Google Analytics, and technical service providers may retain information under their own retention practices. Limited information may also be preserved when necessary to document or respond to a privacy request, opt-out, security issue, legal obligation, or dispute.

3. Personal Information Collected

The personal information processed in connection with the Site depends on how you use it.

3.1 Information You Provide

The quote or contact form may request:

  • your name and job title;
  • your practice, hospital, company, or organization name;
  • your email address and telephone number;
  • your city, state, ZIP code, or other general business location;
  • the number of providers associated with your organization;
  • your practice specialty;
  • the products or services in which you are interested;
  • the contents of your free-text message; and
  • other business or contact information you choose to provide.

Please provide only information necessary for a business quote request. Do not provide patient information, protected health information, financial-account credentials, Social Security numbers, or other sensitive personal information.

3.2 Information Collected Automatically

When you visit the Site, Google Analytics and technical systems may automatically collect information such as:

  • Internet Protocol address and approximate location derived from it;
  • browser type, operating system, device type, and language;
  • pages viewed, links clicked, referring and exit pages, session timing, and interactions with the Site;
  • date and time of access;
  • cookie identifiers, client identifiers, and similar online identifiers; and
  • information about Site performance, errors, security, and traffic sources.

Google Analytics generally uses a first-party cookie, including a cookie commonly named _ga, to distinguish users and sessions. PBS does not use Google Analytics to intentionally collect names, email addresses, telephone numbers, PHI, or other information that directly identifies a person.

3.3 Information from Third Parties

PBS or its service providers may receive limited information from 360Connect concerning the verification, routing, status, or disposition of a quote request. We may also receive aggregated or de-identified reporting concerning Site traffic, quote requests, referral performance, or provider responses.

4. Sources of Personal Information

Personal information may be collected:

  • directly from you when you visit the Site, submit a form, or contact us;
  • automatically from your browser or device through cookies, Google Analytics, server logs, and similar technologies;
  • from 360Connect in connection with form processing, verification, and lead routing;
  • from billing providers concerning the status of a requested quote or referral; and
  • from technical, security, analytics, and hosting service providers.

5. How Personal Information Is Used

Personal information may be used to:

  • receive, verify, administer, and respond to quote requests;
  • match a verified request with as many as five relevant billing providers;
  • enable billing providers to contact you, obtain additional information, and prepare quotes;
  • communicate with you about an inquiry, privacy request, or support matter;
  • operate, maintain, secure, troubleshoot, and improve the Site;
  • measure Site traffic, usage, and referral performance;
  • prevent fraud, spam, abuse, security incidents, and unlawful activity;
  • enforce our Terms and Conditions;
  • comply with law, legal process, and regulatory obligations;
  • establish, exercise, or defend legal claims; and
  • complete a corporate transaction described in Section 7.

PBS does not itself send marketing emails, marketing calls, or marketing text messages after an inquiry. This does not prevent 360Connect from contacting you to verify or facilitate your request, or a billing provider from contacting you in response to the quote request you submitted, subject to the notices presented with the form and applicable law.

6. Paid Lead Disclosure; Sale or Sharing of Personal Information

The Site’s purpose is to connect healthcare service providers seeking quotes with independent medical-billing providers. After a request is verified, the contact and business information in the request may be disclosed to as many as five relevant billing providers. Those providers pay for access to the lead.

Because the providers pay for the lead, this disclosure may be considered a “sale” of personal information under some state privacy laws, even though you intentionally request the referral and the information is disclosed for the purpose of obtaining the quotes you requested. PBS does not sell personal information for unrelated purposes and does not knowingly sell personal information of anyone under 18.

You may opt out of future paid lead disclosures by following Section 12. A request to opt out before a quote request is distributed may prevent the quote-matching service from being completed because disclosure to billing providers is essential to obtaining quotes. An opt-out request will not require a billing provider that already received your information to delete it; you may need to contact that provider directly, although we will assist as required by applicable law.

PBS does not currently use personal information for cross-context behavioral advertising or targeted advertising. If that practice changes, this Privacy Policy and the Site’s preference controls will be updated as required by law.

7. How Personal Information Is Disclosed

Personal information may be disclosed to the following categories of recipients:

  • 360Connect. The form provider processes and verifies quote requests, facilitates matching, and routes verified leads. Its independent practices are described in the 360Connect Privacy Policy.
  • Billing providers. As many as five independent billing providers may receive a verified request and contact you to obtain additional information and prepare a quote. They pay for the lead and process the information under their own privacy policies and business practices.
  • Analytics and technology providers. Google Analytics and providers of hosting, security, infrastructure, and technical support may process information to operate, secure, and measure the Site.
  • Professional advisers. Lawyers, auditors, insurers, accountants, consultants, and similar advisers may receive information when reasonably necessary to provide professional services.
  • Authorities and other parties for legal reasons. Information may be disclosed when reasonably believed necessary to comply with law or legal process; respond to lawful governmental requests; protect rights, safety, security, or property; investigate fraud or misuse; or establish, exercise, or defend legal claims.
  • Transaction participants. Information may be disclosed in connection with an actual or proposed merger, acquisition, financing, reorganization, bankruptcy, sale of assets, or similar business transaction, subject to appropriate confidentiality protections where applicable.
  • At your direction. Information may be disclosed to another person when you direct or consent to the disclosure.

PBS does not control a billing provider’s subsequent use or retention of information after the provider receives a verified lead. Review the provider’s privacy policy and contact it directly with questions or requests concerning its practices.

8. Cookies and Google Analytics

The Site uses cookies and similar technologies for operation, security, functionality, and analytics. Cookies are small files stored on a browser or device. Some cookies may be placed by third parties.

Google Analytics helps measure how users find and interact with the Site. Google may collect device and browser information, approximate location, online identifiers, session statistics, and interaction events. Google processes that information under its own terms and policies. Additional information is available through Google’s explanation of Analytics data collection and Google’s Privacy Policy.

You can manage or delete cookies using browser settings. Blocking cookies may affect Site functionality. You may also use the Google Analytics Opt-out Browser Add-on, where available.

The Site should provide a cookie preference control through which users can manage nonessential analytics cookies. PBS will honor a browser-based opt-out preference signal, such as Global Privacy Control, to the extent required by applicable law and technically supported. Because a quote request is an intentional, user-directed transaction, a browser signal does not replace the form-specific disclosure and choices presented when you request quotes.

9. No Protected Health Information

Do not submit PHI, patient records, patient names, claim-level information, medical record numbers, insurance identifiers, or other patient information through the Site. The Site is not intended or represented as a HIPAA-compliant method of transmitting PHI, and submitting information does not create a business associate relationship between you and PBS.

If prohibited information is submitted, service providers may delete, quarantine, restrict, or otherwise protect it as reasonably appropriate. PBS does not intentionally collect or use consumer health data through the Site.

10. Data Retention

PBS does not maintain a separate database or copy of quote-request submissions. Form information is processed through 360Connect and disclosed to selected billing providers as described above. Those parties determine their own retention periods under their policies, legal obligations, and business needs.

Google determines retention of information it processes through Google Analytics, subject to the configuration selected for the Site and Google’s policies. Technical providers may retain logs and backups for security, operational, or legal purposes.

PBS may retain limited records of privacy requests, opt-outs, complaints, security matters, and legal correspondence for as long as reasonably necessary to honor the request, demonstrate compliance, prevent repeated unwanted disclosures, resolve disputes, or satisfy legal obligations. Aggregated or de-identified information may be retained where it cannot reasonably be linked to an individual.

11. Data Security

PBS and its service providers use reasonable administrative, technical, and organizational safeguards designed to protect information. However, no website, transmission method, or storage system can be guaranteed to be completely secure. You use the Site and submit information at your own risk.

12. Your Privacy Rights and Choices

PBS offers the following privacy choices to individuals in the United States, subject to identity verification, technical feasibility, legal exceptions, and the fact that PBS does not directly retain quote-request records:

  • Access and confirmation: ask whether personal information about you is being processed and request access to available information;
  • Correction: request correction of inaccurate available information;
  • Deletion: request deletion of available personal information, subject to legal exceptions;
  • Portability: request a portable copy of available information where applicable;
  • Opt out of sale or sharing: direct that your personal information not be included in future paid lead disclosures;
  • Opt out of targeted advertising or qualifying profiling: request an opt-out if such processing is used in the future;
  • Withdraw consent: withdraw consent for processing based on consent, without affecting processing that occurred before withdrawal;
  • Appeal: appeal a decision concerning a privacy request; and
  • Non-discrimination: exercise privacy rights without unlawful discrimination.

To exercise a right, email contact@billingsimplified.com with the subject line “Privacy Request” or “Do Not Sell or Share Request.” Describe the right you wish to exercise and provide your name, business email address, organization, and enough information to identify the relevant quote request. Do not send PHI or sensitive identity documents.

We may need to verify your identity and authority before acting. An authorized agent may submit a request where permitted by law, but we may require proof of authorization and direct identity verification. We will respond within the period required by applicable law, generally within 45 days when a state privacy law applies. If we deny a request, you may appeal by replying to the decision with the subject line “Privacy Appeal.”

Because PBS does not directly retain form submissions, we may direct or transmit your request to 360Connect or the billing providers that received the lead where appropriate and legally permitted. You may obtain a more complete or faster response by also contacting those parties directly. PBS cannot guarantee deletion by an independent recipient except where applicable law imposes that obligation.

13. Communication Choices

PBS does not itself send post-inquiry marketing communications. You may nevertheless receive communications from 360Connect to verify or facilitate your request and from billing providers responding to the request.

To stop communications from a particular billing provider, contact that provider directly. A request to PBS may not automatically remove information from an independent provider’s systems. Transactional, legal, security, and privacy-request communications may continue where permitted by law.

14. Do Not Track and Global Privacy Control

Some browsers provide a “Do Not Track” setting. Because there is no uniform industry standard for interpreting that signal, the Site may not respond to Do Not Track signals.

Where required by applicable law and technically supported, the Site will treat a recognized opt-out preference signal, such as Global Privacy Control, as a request to opt out of browser-based sale, sharing, or targeted advertising for the browser or device sending the signal. As noted above, PBS does not currently use personal information for targeted advertising.

15. Third-Party Sites and Services

The Site may link to third-party websites or services. This Privacy Policy does not govern the privacy, security, or other practices of those third parties. PBS is not responsible for their practices. Review the applicable third-party privacy policy before providing information.

16. Children’s Privacy

The Site is intended only for people at least 18 years old. PBS does not knowingly collect, sell, or share personal information from children under 18. If you believe a child has submitted personal information through the Site, contact us so appropriate action can be taken.

17. United States Only

The Site is intended only for users located in the United States. PBS does not intentionally market the Site to or monitor compliance with the privacy laws of other countries. Do not use the Site or submit information if you are outside the United States.

18. Changes to This Privacy Policy

We may update this Privacy Policy from time to time to reflect changes in the Site, vendors, information practices, or law. The revised version will be posted on this page with a new Effective Date. If a change materially affects how previously collected personal information is used or disclosed, additional notice or consent will be provided where required by law.
